I'm familiar with game hosting and port forwarding when using a router. I am assuming this is also required when attempting to connect via 3G (or from Wifi when away from home). Should port 9000 be forwarded to the static IP where the Squeezebox server is running?

And if this is set for the IP address (the IP of the router/modem) will this stream slower than when using the local IP address?

Yes, when away from home you have to setup your router.

Correct, port 9000 should be forwarded to where the server is running. It's only for album art so it is not necessary to do this if you just want audio.

I guess the router forwarding makes it slower, depends on how fast the router is. But this is negligible. The real slowdown is the 3G connection.

Therefore I only need to use the external IP address of my modem to connect even if I am using dynamic IP address on the server PC?

If you use dynamic IP you must connect to whatever external IP address is assigned to your modem at that time.
Even if you open port 9000 for album art, you still only connect to that external IP address.
